Reagent Composition:
Glucose-dye-oxidoreductase* - 1.0 U
Bis-(2-hydroxyethyl)-(4-hydroximinocyclohexa-2,5-dienylidene)-ammonium chloride - 9.8 ug
2,18-Phosphomolybdic acid - 127.0 ug
Stabilizer - 0.25 mg
Nonreactive substances - 2.6 mg
Directions:
Store at room temperature between 36°F (2° C) and 86°F (30° C). Do not freeze.

ACCU CHEK SAFE T PRO DESCRIPTION
Accu Chek Safe T Pro Blood Glucose Lancets
Completely disposable, easy-to-use lancets. Designed for single use only.
The Accu-Chek Safe T Pro lancets are designed to minimize health risks to all exposed to blood borne- pathogens with unique Safe-T-Wing tabs that prevent the device from being reused. The lancet also permanently retracts into its protective case after use to prevent accidental fingersticks during disposal.
Directions:
1. Hold Accu-Chek Safe-T-Pro lancet and twist off blue protective lancet cap. (Note: Do not use lancet if protective cap has been previously removed.)
2. Firmly press the Accu-Chek Safe-T-Pro lancet against the fingertip and press the blue button.

ACCU CHEK SOFTCLIX DESCRIPTION
Accu Chek Softclix Lancets
The lancet that minimizes discomfort. Uniquely designed for accurate penetration. Use only with the Accu-Chek Softclix device.
Directions:
1. Unscrew the cap from the Accu-Chek Softclix device.
2. Insert a new Accu-Chek Softclix lancet in the lancet holder. It is properly positioned when you feel it snap into place. To remove the protective cap, twist and pull it away form the lancet.
3. Screw the cap onto the device and turn the cap clockwise until you cannot turn it any farther.
4. Wash your hands in warm, soapy water. Dry well.
5. Hold the Accu-Chek Softclix device against the side of your fingertip.
6. Press the release button.
7. Apply the drop of blood onto the target area of your test strip.
8. Immediately remove used lancet. To avoid accidental injury, do not store lancet in device.
Caution
Do not use if protective guard has been previously removed from lancet. Do not reuse lancets. Do not store lancet in device. A restricted license limits use of the Accu-Chek Softclix lancet device. Carefully read the package insert before use.
