SULFINPYRAZONE DESCRIPTION

Sulfinpyrazone - Tablet

Sulfinpyrazone (sul-fin-PEER-a-zone ) is used in the treatment of chronic gout (gouty arthritis), which is caused by too much uric acid in the blood. The medicine works by removing the extra uric acid from the body. Sulfinpyrazone does not cure gout, but after you have been taking it for a few months it may help prevent gout attacks. This medicine will help prevent gout attacks only as long as you continue to take it. Sulfinpyrazone is sometimes used to prevent or treat other medical problems that may occur if too much uric acid is present in the body.

How does sulfinpyrazone work? What will it do for me?

Sulfinpyrazone is used for treatment of chronic gout and for certain blood clotting disorders. It is also sometimes used after a heart attack to reduce the risk of another attack. It works by decreasing blood clotting and preventing gout crystals from forming.

Your doctor may have suggested sulfinpyrazone for conditions other than the ones listed in these drug information articles. If you have not discussed this with your doctor or are not sure why you are taking sulfinpyrazone, speak to your doctor. Do not stop taking sulfinpyrazone without consulting your doctor.

How should I use sulfinpyrazone?

Sulfinpyrazone is often started at lower doses (e.g., 100 mg twice daily) and increased gradually up to 800 mg daily (e.g., 200 mg four times daily), depending on the needs of the person. Sulfinpyrazone may be taken with or without food. If the medication upsets your stomach, you may take it with food. If you still experience problems, you may take the medication with an antacid. Your doctor may want you to drink up to 8 to 10 glasses of water daily when you first start taking sulfinpyrazone. It is important that you follow your doctor's directions carefully to avoid the formation of acidic urine.

Many things can affect the dose of medication that a person needs, such as body weight, other medical conditions, and other medications. If your doctor has recommended a dose different from the ones given here, do not change the way that you are taking the medication without consulting your doctor.

It is very important that sulfinpyrazone be taken exactly as prescribed. Finish all of the prescribed medication even if you start feeling better. If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your usual dosing schedule. Do not double doses.

Are there any other precautions or warnings for sulfinpyrazone?

Allergies: If allergic skin reactions occur, stop taking sulfinpyrazone.

Fluids: If you are prone to gout (high uric acid levels) and also take diuretics (water pills), make sure to drink lots of fluids. Your doctor may want you to start with a low dose of sulfinpyrazone. At the start of treatment, attacks of gout may occur more frequently because uric acid is being freed from affected joints. Therefore, maintain an adequate fluid intake. If an attack of gout occurs, sulfinpyrazone should be continued but your doctor will likely add other treatments (e.g., colchicine or nonsteroidal anti-inflammatory drugs (but not ASA)).

Kidney function: Those with reduced kidney function should use sulfinpyrazone with caution. Sulfinpyrazone may lead to sodium and water retention.

Stomach problems: People with a history of stomach problems or a healed peptic ulcer should use sulfinpyrazone with caution.

Pregnancy: The safety of sulfinpyrazone for pregnant women has not been established. Sulfinpyrazone should not be used during pregnancy, especially in the first trimester, unless, in the opinion of the doctor, the expected benefits outweigh the potential risks.

Breast-feeding: It is not known whether sulfinpyrazone passes into breast milk. If you are taking sulfinpyrazone and plan to breast-feed, the potential benefits must be weighed against the potential risks.

What other drugs could interact with sulfinpyrazone?

The following medications may affect the way that sulfinpyrazone works or may increase the risk of side effects:

carbenicillin (by injection)
cefamandole
cefaperazone
cefotetan
dipyridamole
divalproex
heparin
nonsteroidal anti-inflammatory drugs (NSAIDs; e.g., ibuprofen, naproxen)
entoxifylline
plicamycin
salicylates (e.g., ASA, salsalate)
ticarcillin
valproic acidwarfarin
Sulfinpyrazone may affect how the following medications work or increase their risk of side effects:

acetaminophen
nitrofurantoin
tolbutamide
warfarin
If you are taking any of these medications, speak with your doctor or pharmacist. Depending on your specific circumstances, your doctor may want you to:

stop taking one of the medications,
change one of the medications to another,
change how you are taking one or both of the medications, or
leave everything as is.
An interaction between two medications does not always mean that you must stop taking one of them. In many cases, interactions are intended or are managed by close monitoring. Speak to your doctor about how any drug interactions are being managed or should be managed.

Medications other than those listed above may interact with sulfinpyrazone. Tell your doctor or prescriber about all prescription, over-the-counter (non-prescription), and herbal medications that you are taking. Also tell them about any supplements you take. Since caffeine, alcohol, the nicotine from cigarettes, or street drugs can affect the action of many medications, you should let your prescriber know if you use them.
